Withdrawal from the College Form

Withdrawing from the College may have both academic and financial implications.  Once a withdraw request has been processed, it becomes part of your academic record and it is non-reversible.

To withdraw from a Mount Holyoke College graduate program you must:
  • Make an appointment for a closing interview with the Executive Director of the Professional and Graduate Programs.
  • Review the course catalog for college refund policies and set up a meeting to discuss financial implications with a representative of Student Financial Services.
  • Complete and submit this form.
NOTE: Official notification of the your intention to withdraw does not occur until this form is processed by the Office of the Registrar.  Attending a closing interview does not constitute notice of intent to withdraw or official withdrawal by you.  Correspondence from Mount Holyoke College will be through your Mount Holyoke email address or by mail to the last permanent address noted on your student record.
